Low-Dose Radiation Therapy for Plantar Fasciitis
One of the most-studied non-joint conditions treated with LDRT
Low-dose radiation therapy (LDRT) is an outpatient treatment that directs a very small amount of radiation at an inflamed area, aiming to calm the inflammation that drives chronic pain. Plantar fasciitis, sometimes called painful heel spur, is one of the conditions with the longest track record of LDRT use, particularly in Germany and other parts of Europe.
What the Evidence Shows
A German multicenter randomized trial compared a standard radiation schedule against a lower-dose schedule for painful heel spur, following patients for 12 months to assess how well pain relief held up over time.[1] A separate prospective Swiss study followed patients with plantar fasciitis, along with epicondylitis and finger osteoarthritis, and found meaningful pain relief at rest and during activity, along with improved walking time, with benefits maintained at 12 months of follow-up.[2]
Retrospective case series have generally reported pain relief in roughly 65% to 90% of patients treated for plantar fasciitis, with response commonly cited around 80% to 85% in more recent summaries.[3] A 2026 clinical review covering LDRT for musculoskeletal conditions described the evidence for plantar fasciitis as among the stronger data sets supporting LDRT's anti-inflammatory use outside of osteoarthritis itself.[4]
Typical Schedule
- Total dose of about 3 to 6 Gy, depending on the program's protocol, split into 6 treatments
- Treatments given 2 to 3 times per week, over about 2 to 3 weeks
- Each visit takes roughly 10 to 15 minutes; no needle, sedation, or recovery period
Who May Be a Candidate
Programs generally consider adults with chronic plantar fasciitis, typically lasting more than 3 to 6 months, who have already tried conservative treatments such as stretching, orthotics, physical therapy, or corticosteroid injections without lasting relief. Most published studies enrolled patients over the age of 40. As with other LDRT applications, a radiation oncologist weighs age, duration of symptoms, and prior treatment history before recommending it.
